This e-learning module is an intermediate curriculum which delves into concepts that address a successful transition to home, the community, and beyond. It utilizes an interdisciplinary approach to care that spans both the inpatient and outpatient rehabilitation settings. Through evidence-based research, case studies, knowledge checks, and videos, participants will gain a deeper understanding of the various ways that they can help support and prepare their patient in reintegrating back into the community.
